Key/Essential Functions & Responsibilities: Assists in the completion of assessments and plans for each person in the home, obtaining, developing and recording the information necessary for the completion in person or remotely and ensuring compliance with all applicable regulations and timelines. Completes intake assessments during the referral process, reviewing current and historical records and documentation, performing direct observations and interviews with the individual and their support group to determine support needs and services required for success. Participates in the transitional planning process working closely with the Qualified Behavior Modification Professional (QBMP), including Individual Education Plan (IEP), Individual Program Plan (IPP), Individual Behavior Support Plan (IBSP) meetings and seeks out community resources that will best support individual needs. Completes a well-rounded and appropriately documented Needs and Services Plan to support each individual and ensures that each plan is regularly reviewed and updated as often as necessary, but at least every six (6) months, to ensure its accuracy and to document significant occurrences that result in changes in the client's physical, mental, psychological, and/or social functioning. Collaborates with QBMP and interdisciplinary team to ensure continuity of care for discharge and coordinates services accordingly. Acts as a liaison between the home and various parties on behalf of the people we support including their conservator(s) and/or advocates; oversight and social services agencies (CA Department of Developmental Services, Department of Social Services – Community Care Licensing Division and Regional Centers); and other entities such as hospitals, court systems, day programs and/or school districts to coordinate and support service delivery. Maintains and appropriately documents each person’s services records in accordance with appropriate California statutes and regulations, specifically Titles 17 and 22. Ensures monthly recommendations, notes and service hours for all persons supported are appropriately documented for billing purposes using a digital platform (Digital Agency, etc.). Frequently attends and participates in meetings related to the service and support needs of assigned individuals in the home and regularly meets with persons supported to obtain feedback regarding plans and services and to evaluate progress. Attends meetings related to assigned individuals and CA YAI-wide meetings, including external or agency oversight meetings relating to the person YAI supports. Provides and documents training for group home staff regarding support needs of persons moving into the home including their Needs and Services plans and discharge plans; regularly conducts additional trainings as requested by Program Administrator(s) or clinical supervisor and/or as needs and services change. Conducts regular Residential Council Meetings including the people supported, updating the appropriate program staff on the proposed monthly menu and activity schedule at least once per month. Regularly works in partnership with the Program Operations team to keep informed and assist with service planning and new program development and collaborates weekly with the Registered Nurse (RN), QBMP, Administrator and group home staff supporting people with their monthly schedules and appointments. Builds and maintains positive relationships with all levels of staff and the people we support. May provide education and awareness to people supported regarding feedback received, including addressing concerns and requests for changes to services that may be impractical/unsafe, involving appropriate subject matter experts (e.g. RN, Nutritionist, etc.).
