About The Position

Requirements

  • Bachelor's Degree in Social Work or another Human Services field with at least three years of social work experience.
  • Hold or obtain CPR/First Aid, Ukeru, and CPI certifications.
  • Knowledge of California’s Lanterman Act and related service systems.
  • Highly organized and flexible with the ability to manage multiple priorities.
  • Exceptional verbal and written communication skills.
  • Proficiency in technical systems such as Behavior Agent, Workday, and Microsoft Teams.
  • Ability to work in a stressful environment.
  • Ability to meet essential physical demands of the position.

Nice To Haves

  • Master’s Degree in Social Work or a related discipline.
  • At least six months prior experience providing direct care to individuals with developmental disabilities.
  • Knowledge and awareness of I/DD or mental health needs.
  • Familiarity with residential services for people with I/DD in California.

Responsibilities

  • Assist in the completion of assessments and plans for each person in the home.
  • Complete intake assessments during the referral process.
  • Participate in the transitional planning process with the Qualified Behavior Modification Professional (QBMP).
  • Complete and regularly update Needs and Services Plans for individuals.
  • Collaborate with the QBMP and interdisciplinary team for continuity of care.
  • Act as a liaison between the home and various parties on behalf of the individuals supported.
  • Maintain and document service records in accordance with California statutes and regulations.
  • Document monthly recommendations and service hours for billing purposes.
  • Attend and participate in meetings related to service and support needs.
  • Provide and document training for group home staff.
  • Conduct regular Residential Council Meetings.
  • Work in partnership with the Program Operations team for service planning.
  • Build and maintain positive relationships with staff and individuals supported.
