About The Position
Crown Heights Center for Nursing and Rehabilitation is looking for a Social Worker / Geriatric Behavioral Health Specialist to join the team at our Skilled Nursing Facility in Brooklyn, NY! Who We Are The Allure Group is a consortium of six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post-acute and sub-acute skilled nursing and rehabilitation. Requirements
- Bachelor’s degree in social work, Psychology, Counseling, or related field
- Knowledge of behavioral interventions for dementia and other age-related conditions
- Strong clinical assessment and documentation skills
Nice To Haves
- Previous experience working with geriatric populations, especially in long-term care or SNF settings.
Responsibilities
- Conduct behavioral health assessments for new and existing residents.
- Develop and implement individualized behavioral care plans in collaboration with the interdisciplinary team.
- Provide counseling, crisis intervention, and therapeutic support for residents experiencing anxiety, depression, dementia-related behaviors, or other psychiatric issues.
- Offer education and support to staff and families on behavior management techniques and mental health awareness.
- Collaborate with medical, nursing, social work, and activities teams to ensure holistic resident care.
- Monitor and document progress and adjust treatment plans as needed.
- Participate in care planning meetings and regulatory surveys.
