About The Position
Liquid Personnel is recruiting an Adult Social Worker to join its client’s Generic team, based in Darlington.
Requirements
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
Responsibilities
- Facilitate safe and timely hospital discharges using intermediate care or long‑term support services, ensuring a smooth “home first” transition.
- Complete person‑centred, strengths‑based assessments and ongoing reviews to support recovery and determine longer‑term care needs.
- Work both independently and within the RIACT multidisciplinary team while maintaining high‑quality case recording and applying the Care Act 2014 and discharge procedures.
