Responsibilities

• Collaborate with the healthcare team to develop treatment plans and identify appropriate resources.
• Provide psychosocial services as identified in the patient's comprehensive plan of care.
• Educate patients, families, and healthcare team members about treatment options and self-management support.
• Coordinate family-centered care team meetings and provide crisis assessment and interventions.
• Act as a liaison between patients, families, and the healthcare team, advocating for their needs.
• Facilitate communication and coordination among healthcare team members and patients/families.
