Requirements

  • Minimum of 2 years' experience in medical social work or case management.
  • Active and unrestricted LCSW, LSW or LMSW in AR.
  • Ability to travel 50-75% within Little Rock, AR.

Responsibilities

  • Participates in the care planning process in collaboration with the Care Manager, including assessment, goal setting, establishing interventions, identifying barriers, monitoring success, and evaluating the overall care plan.
  • Focuses on quality of care, compliance, outcomes, and decreasing costs in care management activities.
  • Develops and carries out strategies to coordinate and integrate post-acute and long-term care services to members.
  • Performs initial and periodic assessments of members enrolled in the Long-Term Care Program.
  • Applies social work concepts and strategies to address social determinants of health in members' care plans.
  • Conducts regular discussions and updates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff regarding member status and progress towards goals.
  • Serves as a member advocate to ensure necessary care is received under the member's benefit plan and available community resources.
  • Develops relationships with hospital social workers and community resources to assure appropriate care management of members.
  • Assists in identifying and reporting potential quality improvement issues.
  • Directs social work interventions including psychosocial assessments, documentation of problems, and promoting access to a continuum of care.
