  • Participates in the care planning process in collaboration with the Care Manager.
  • Conducts care management activities through telephonic and face-to-face interactions.
  • Focuses on quality of care, compliance, outcomes, and decreasing costs.
  • Develops and carries out strategies to coordinate and integrate post-acute and long-term care services.
  • Performs initial and periodic assessments of members enrolled in the Long-Term Care Program.
  • Applies social work concepts, principles, and strategies in addressing social determinants of health.
  • Conducts regular discussions and updates with providers and care management staff.
  • Serves as a member advocate to ensure necessary care is received.
  • Develops relationships with hospital social workers and community resources.
  • Assists in the identification and reporting of potential quality improvement issues.
  • Directs social work interventions including psychosocial assessments and documentation.
  • Minimum of 2+ years' experience in medical social work or case management.
  • Active and unrestricted LCSW or LMSW in CT.
  • Ability to travel 50-75% within Bridgeport, CT.
  • Master's degree must be in Social Work.
