About The Position

Richland Place is looking for a Social Worker/Discharge Planner to join our team! The qualified applicant for this position should hold a Bachelor's degree or Masters in social from an accredited program or a related degree in sociology or psychology. Experience preferred with a case management background in a SNF / ICF setting, knowledge of MDS and clinical assessments and documentation, care planning, discharge planning, working with support agencies such as hospice, home health and mental health services, etc.

• Experience in care planning and discharge planning
• Ability to work with support agencies such as hospice, home health, and mental health services

Responsibilities

• Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
• Working with individual patients and their representatives toward transition back to the community or adjustment to center life and helping navigate crisis of illness and disability
• Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
• Coordinating patient's plan of care with interdisciplinary team members
• Reporting to the Director of Social Work current case management status and needs
• Maintaining confidentiality of professional information
