About The Position

Assists physician and health care team members in understanding the significant social and emotional factors related to health problems; participates in the development of the plan of treatment; prepares clinical and progress notes; works with the family; utilizes appropriate community resources; leads patient groups; and participates in discharge planning. This position will be assigned to the Behavioral Health Unit and work with both the geriatric and adult populations.

Requirements

• Bachelor's degree in social work required, Master's preferred.
• South Carolina license in Social Work required
• Knowledge regarding community resources required.
• One year of hospital/health care experience preferred.
• Excellent communication skills and ability to work in a collaborative team approach required.
• Master's Degree in Counseling or related field.
• LPC or LMFT required.
