About The Position
The Social Worker for Community Health Initiatives plays a crucial role in providing professional services to clients and their families who are eligible for Healthy Beginnings Services. This position focuses on addressing psychosocial, emotional, financial, and environmental needs through assessments, counseling, and resource referrals. The social worker will also engage with patients and families aboard the Mobile Health Clinic, ensuring comprehensive support and care transitions through an interdisciplinary team approach.
