About The Position

As part of the Telecare Family, Willow Rock is a 16-bed Psychiatric Health Facility in Alameda County for adolescents 12-17 years old who are experiencing acute psychiatric symptoms. Shifts Available: On Call; Shift Hours and Days vary as needed. Expected starting wage range is $24.95 - $28. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Requirements

  • A Bachelor's Degree in Social Work or psychology from an accredited college or university.
  • Demonstrated experience with behavioral health services.
  • Must be at least 18 years of age.
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment.
  • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification.
  • If the position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment.

Responsibilities

  • Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders.
  • Identifies and plans for psychosocial needs of members served.
  • Works with members served and multidisciplinary treatment teams to design, implement, and evaluate the Recovery Plan.
  • Provides education to members served and family members regarding mental health conditions and treatment options.
  • Has responsibility for discharge planning, including assessing readiness for discharge of members served in coordination with the recovery/treatment/service team.
  • Makes appropriate referrals for placement, case management, and aftercare services, as assigned.
  • Coordinates with the Business Office to ensure that benefits and finances of members served are in order, as assigned.
  • Develops and maintains an awareness of community resources.
  • Assures that members served know and understand their legal status and rights; tracks legal status on all assigned members served.
  • Provides consultation to members served and their families regarding issues arising from the admission, discharge, and/or aftercare plan.
  • Must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training.
