Summary: The provision of these job responsibilities are mandated by the Centers for Medicare & Medicaid (CMS) for Medicare-approved transplant centers. As a member of the transplant team, the Social Worker utilizes specialized clinical knowledge and advanced clinical skills to help in the areas of assessment, treatment of medical, patient/ caregiver navigation, mental health and behavioral disorders, and discharge planning. This position involves providing behavioral health support and crisis intervention for children, teens, and adolescents experiencing emotional or mental health challenges. The Licensed Social Worker will work three 12-hour shifts per week with rotating weekends. The ideal candidate will have demonstrated experience in crisis management, behavioral health care, and therapeutic support, along with strong communication, assessment, and de-escalation skills.

Requirements

  • Masters: Social Work (Required)
  • Licensed Clinical Social Worker (SW LICENSE) - State of Florida (FL)
  • Complexity of Work: Requires critical thinking skills, effective (verbal and written) communication skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful and fast paced environment and take appropriate action. Must have strong organizational/priority setting/multi-tasking skills. Must be able to learn and apply large amounts of technical and procedural information.
  • Minimum of two (2) years' post MSW experience in related field required.
  • Previous experience working in hospital setting.

Nice To Haves

  • On-going clinical transplant education required.
  • Participation in Quality Assurance and Performance Improvement (QAPI).

Responsibilities

  • Participates in transplant selection committee meetings.
  • Provides crisis intervention for patients and family members.
  • Interviews and assesses patients and/or family, caregivers, and/or legal representatives.
  • Performs comprehensive psychosocial assessments for pre- post- and living donors to determine transplant candidacy and continue to work with patient and family through all phases of the transplant process during inpatient and outpatient and end of life.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • May supervise and/or train new staff, students, and interns.
  • Attends multidisciplinary rounds as mandated by CMS for all Medicare-approved transplant centers and identifies appropriate resources for financial, insurance, transportation, housing, healthcare, mental health, spiritual services, and education about diagnosis and transplantation.
  • Assists patient in the comprehension of complex instructions, resolves conflicts, or facilities conflict resolution.
  • Develops behavioral agreements with patient and/or families that address adherence and commitment to treatment plan.
  • Assists with patient fundraising.
  • Designs and facilitates support groups and other programs.
  • Consults and collaborates with multidisciplinary transplant providers as mandated by CMS for all Medicare-approved transplant centers to promote, monitor, and evaluate patient's ability to understand and adhere to treatment plan and lifestyle changes.
  • Makes appropriate referrals to community agencies, such as DCF, APS, SSA, CMS, disease specific resources.
