About The Position

Performs difficult work scheduling and conducting assessments of client needs, conducting home visits and relative searches, guiding clients through offered services and providing referrals, coordinating and planning services, testifying in court and providing documentation, providing case consultation, completing required reports, and related work. Work is performed under the limited supervision of Social Services Supervisor.

Requirements

  • Knowledge of State and Federal laws that affect Adult Protection, Developmentally Disabled, Vulnerable Adults, Child Protection and child Welfare ability services.
  • General knowledge of mental health issues.
  • Knowledge of the principals, methods, and techniques of social work.
  • Ability to interview people to obtain accurate information.
  • Ability to mediate between people in conflict.
  • Knowledge of system theory, community services.
  • Skills in interpersonal relations.
  • Knowledge of the dynamics of individual and group behavior.
  • Knowledge of human growth and development.
  • Knowledge of social and economic problems
  • Ability to analyze complex situations, negotiate for case planning and evaluate for potential improvement.
  • Ability to present complicated case information clearly and concisely.
  • Ability to organize work effectively.
  • Ability to maintain good communications with clients, co-workers, and community organizations.
  • Ability to work under stressful conditions and to remain calm and objective.
  • Skill in recordkeeping and skill in oral communications.

Responsibilities

  • Complete child protection assessments, as assigned.
  • Assess need for services to families and children.
  • Provide emergency services, placement or other services as needed.
  • Complete CHIPS petitions as needed and present in court process for agency.
  • Develop social service plans with families.
  • Monitor on-going status of children and families receiving services.
  • Do follow up home visits and monitoring of services provided.
  • Monitor contract service provision.
  • Develop and implement placement plans and visitation schedules when appropriate
  • Maintain case records according to agency procedures.
  • Re-assess case plan regularly and identify need of on-going services.
  • Determine type of out-of-home placement needed.
  • Develop and implement appropriate follow up and referral plans.
  • Close cases when appropriate in a timely fashion.
  • Complete an assessment, develop and implement a crisis plan and individual, family, community support plan.
  • Assess, reassess and evaluate the delivery, appropriateness and effectiveness of services over time.
  • Do minimally one face to face visit per month, telephone contact as needed.
  • Assist the child and family in obtaining needed services by coordination with other agencies involved with the child and/or family to assure continuity of care.
  • Assistance and outreach to other family members in obtaining a diagnostic assessment, as needed, for determination of mental health case management services.
  • Work with Child & Teen Check-up Program.
  • Participate in training.
  • Arrange for and develop staff development workshops as needed.
  • Receive other work assignments as requested.
  • Complete services as needed.
  • Make case presentations to appropriate review/authorization committees.
  • Provide afterhours services.
