How you’ll make an impact in this role Lead comprehensive initial assessments for ICU admissions, identifying psychosocial factors that will influence the patient’s recovery and long-term care needs from day one. Advocate for patients experiencing mental health crises by securing appropriate psychiatric placements and resources, ensuring their safety and specialized care during recovery. Support families navigating the emotional weight of critical illness, providing expert guidance and crisis intervention to help them make informed decisions during high-stress transitions. Collaborate with the ICU interdisciplinary team to determine the "next site of care," expertly coordinating transfers to rehabilitation, long-term acute care, or home to ensure clinical continuity. Create a seamless discharge experience by resolving complex financial or social barriers, ensuring patients have the support systems required to prevent ICU readmission.
