Assess patients' needs and coordinate the overall interdisciplinary plan of care from admission to discharge within Case Management departments. Provide support and education regarding resources available to cope with emotional, financial and social needs that arise with a diagnosis. Develop care plans for patients based on assessment findings and consideration of needs and preferences. Assess complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support. Advocate for patients' needs and ensure the medical system respects their rights and caters to their preferences. Maintain ongoing dialog with leadership and care transition team members to ensure effective implementation and reevaluation of care plan.
