Functions as the key member of the health care team responsible for ensuring appropriate discharge planning ensured through assessment, (social, financial, and health conditions of patient) from which recommendations for referrals, services, and post hospital care will be provided. This function will be in collaboration with physicians, nursing staff, physical therapy and/or other members of the multi-disciplinary team as appropriate.
