How you’ll make an impact in this role Assess child development stages and family dynamics to create specialized care plans that support both the young patient’s growth and the family’s ability to provide care. Coordinate between medical teams, schools, and community agencies to ensure a seamless transition from the hospital to a supportive home and educational environment. Advocate for the unique rights of children and their guardians to ensure treatment plans are developmentally appropriate and respect the family's values. Support families through crisis intervention and emotional guidance, helping them navigate the trauma of a child’s diagnosis and build long-term resilience. Empower caregivers by connecting them to specialized pediatric resources, such as financial aid or developmental therapies, to reduce the burden of long-term care management. What minimum requirements you’ll need Licensure / Certification / Registration: Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Certified Social Worker preferred. Education: Bachelor's degree in Social Work required. Work Experience: 3 years of experience preferred. Requirements

  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
