The Social Worker conducts comprehensive psychosocial patient assessments for early identification of barriers to facilitate timely discharge planning. Coordinates inpatient discharge planning, collaborates with interdisciplinary team members, assists patients and/or families with continuing post-acute care needs, facilitates crisis intervention, provides emotional support and resource information, arranges patient/family case conferences, and facilitates ethics consultations.

  • Conducts timely initial and discharge psychosocial assessments.
  • Consistently documents concurrent social work interventions to address the discharge plan of care in the medical chart.
  • Maintains compliance with State and Federal regulatory requirements to include CMS Conditions of Participation for discharge planning; issuing discharge IM letters, CMS Choice.
  • Works independently, seeking consultation when needed and keeping supervisor apprised of patient/program developments.
  • Collaborates and completes referrals to appropriate home health agencies, skilled nursing facilities, inpatient rehabilitation facilities, long-term acute care hospital(s), post-acute care provider(s) and other facilities and community resources.
  • Facilitates communication between the patient, families and interdisciplinary health care team.
  • Coordinates and directs availability of resources to service identified needs of patients.
  • Maintains a stated level of quality while minimizing resource usage and decreasing length of stay.
  • Provides social work coverage for the hospital evenings, weekends and holidays as assigned.
  • Assumes other duties and responsibilities that are related and appropriate to the position.
  • Up to date on CMS guidelines.
  • Communicating with doctors, nurses, home health companies and other healthcare personnel and agencies.
  • Coordinating healthcare services in the community.
  • Understanding various medical conditions and terminology.
  • Evaluating the mental, emotional and physical status of patients.
  • Adjust treatment plans in compliance with changes in the status of the patient.
  • Keeps abreast of current developments in clinical social work and health care.
  • Maintains social work standards required by various licensing, accrediting and quality assurance review bodies.
  • Experience as a licensed Social Worker desired.
