About The Position

Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Requirements

  • Bachelor's degree in Social Work required
  • Master's degree preferred

  • Minimum two years in acute care, SNF, physician offices, or community health settings

  • LBSW license in Maryland required
  • Must obtain care/case management certification within five years of hire

  • Strong communication and multitasking skills
  • Familiarity with EMRs and ability to navigate electronic medical records
  • Knowledge of disease processes, regulatory requirements, and evidence-based care

Responsibilities

  • Coordinate psycho-social support for patients during hospitalization
  • Develop and implement personalized discharge plans
  • Assess patient needs related to care coordination, resources, and services
  • Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction
  • Facilitate access to community resources to support recovery and continuity of care
  • Ensure compliance with hospital policies, regulatory standards, and evidence-based practices
