As a Social Worker on our team, you will apply your expertise in crisis intervention, resource navigation, mediation, counseling, and patient empowerment to support individuals and families from diverse backgrounds and with varying needs. Requirements

  • Master’s Degree in Social Work from a CSWE accredited program.
  • Current state licensure as a Licensed Social Worker (LSW).
  • State licensure as a Licensed Independent Social Worker (LISW) within five years of hire.
  • Two years of experience in a hospital or healthcare setting.

Responsibilities

  • Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
  • Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings).
  • Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
  • Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
  • Maintain a network of resources for consultation and connect patients with community resources.
  • Supervise Social Work Interns and assist with training new caregivers.
  • Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
