About The Position
The position involves utilizing clinical judgment and expertise to coordinate care management and behavioral health interventions. The role requires developing strategies to ensure cost-effective, quality care outcomes for patients with complex care needs. Responsibilities include assessing, planning, monitoring, and providing therapeutic interventions, as well as collaborating with patients and families to facilitate understanding of diagnoses and support necessary lifestyle and behavior changes. The position also involves responding to crisis situations and engaging with patients and families to access appropriate levels of care across the continuum. Emphasis of duties may vary depending upon the area of assignment.
