Cleveland Clinic's Social Work team provides caring, patient and family-focused social work services to meet basic human needs, enable clients to reach their optimal level of psychosocial functioning and enhance well-being. In this role, you will coordinate the transition of care to help patients move through the hospital and ensure that they have everything they need to make the transition home. A caregiver in this position works days from 7:00AM - 3:30PM, including rotating weekends and holidays.

Requirements

  • Bachelor's Degree in Social Work (BSW) from an accredited school of Social Work
  • Licensed Social Worker (LSW) in the current state
  • Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross
  • Knowledge of and practice in accordance with the NASW Code of Ethics

Nice To Haves

  • Three years of experience in healthcare systems to include exposure to and experience in pre-acute and post-acute care
  • Social Worker or Case Management Certification (CCM) within one year of eligibility
  • Hospital social work experience
  • Knowledge of community resources

Responsibilities

  • Assist patients and families in achieving improved health outcomes by reducing the bio psychosocial spiritual obstacles that interfere with their ability to use health resources effectively.
  • Assess patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
  • Assess patient and family's financial, social, emotional, educational and environmental resources and identify needs based on patient's current medical condition.
  • Develop an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patient's medical condition.
  • Participate in discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.
