Cleveland Clinic Union Hospital is looking to add a Social Worker to the team who will work with patients in the Emergency Department and Med/Surg floor. As a Social Worker, you will assist patients and their families in achieving improved health outcomes by reducing obstacles that interfere with their ability to use health resources effectively. This team works hard to connect patients with needed services in the community and sets them up for a successful discharge to prevent readmission. In this role, you will feel as though you are making a difference in each patient's life and be a part of a family-oriented team that treats our patients as our own family members. A caregiver in this position works days from 7:00AM - 3:30PM or 7:30AM - 4:00PM, including rotating weekends and one holiday per year.

Requirements

  • Bachelor's Degree in Social Work from an accredited school of Social Work
  • Licensed Social Worker (LSW) in the current State
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) or the American Red Cross
  • Knowledge of and ability to practice in accordance with the NASW Code of Ethics
  • Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes
  • Flexible, open-minded and adaptable to change
  • Requires normal or corrected hearing and vision to normal range.
  • Ability to perform work in a stationary position for extended periods.
  • Ability to communicate and exchange accurate information
  • Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment.
  • Ability to travel throughout the hospital system.
  • Requires normal or corrected hearing and vision to normal range.
  • In some locations ability to lift up to 10 pounds

Nice To Haves

  • Three years of experience in healthcare systems to include exposure to and experience in pre-acute and post-acute care
  • Social Worker OR Case Management Certification (CCM) within one year of eligibility
  • Hospital Social Work experience
  • Knowledge of community resources

Responsibilities

  • Assess patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
  • Assess patient and family's financial, social, emotional, educational, and environmental resources and identify needs based on patient's current medical condition.
  • Develop an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patient's medical condition.
  • Participate in Discharge planning activities for complex patients to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.
