The position involves planning, organizing, coordinating, and implementing comprehensive social work services to patients in the assigned setting. The goal is to ensure appropriate and timely discharge planning and provision of services that address the social, emotional, economic, and environmental needs affecting the patient's medical condition and/or progress. The social worker will develop and implement plans of care based on identified needs.
