About The Position

The Social Worker (BSW) at Wesley Medical Center is responsible for promoting patient-centered care by evaluating psychosocial needs and coordinating community services for identified patient populations. This role involves working collaboratively with a multidisciplinary health team to facilitate discharge plans and ensure smooth transitions across the continuum of care, primarily within Med/Surg units.

Requirements

• Bachelor's Degree in Social Work (BSW) or Licensed Baccalaureate Social Worker (LBSW).
• Basic Cardiac Life Support certification must be obtained within 30 days of employment start date.

Responsibilities

• Evaluate psychosocial needs of patients and provide community service coordination.
• Act as a liaison between patients and community resources/post-acute care providers.
• Create and facilitate timely implementation of discharge plans.
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual factors.
• Perform thorough assessments and develop appropriate discharge plans for high-risk patients.
• Reassess and modify discharge plans in conjunction with the clinical team.
• Participate in interdisciplinary care planning and proactive discharge planning.
• Educate patients and caregivers about community resources and post-acute care criteria.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers.
• Assist patients and families in making difficult decisions while honoring their preferences and values.
