As a Hospice Social Worker - Volunteer Coordinator, You Will: Provide psychosocial support to patients and families in accordance with their individualized plan of care Conduct social work assessments and provide education, case management, and care planning services Offer emotional and resource-based support related to family relationships, cultural/spiritual beliefs, long-term care, and advance care planning Monitor and support safe home environments for hospice patients Serve as the key liaison and advocate for all volunteer services within your branch Assess patient and family needs for volunteer support and assign volunteers accordingly Supervise, train, and support volunteers, ensuring files, documentation, and visit logs are current and compliant Plan and facilitate ongoing volunteer education, recognition, and engagement
