About The Position

As a Social Worker at Northfield Hospital + Clinics, you will provide essential services to meet the psychosocial needs of patients and their families. This role involves conducting assessments, planning discharges, offering emotional support, and connecting families with necessary resources. The position is part-time, focusing on delivering high-quality care in a fast-paced environment.

Requirements

• Licensed by the Minnesota State Board of Social Work, Master's level preferred.
• 2 years of social work experience in a health care environment preferred, including hospital or nursing home.

Nice To Haves

• Bilingual skills are preferred.

Responsibilities

• Provides psychosocial assessment for patients and their families to determine needs and appropriate services for all units of the facility including medical/surgical, emergency room, birth center, and clinic.
• Provides social work services including supportive counseling, discharge planning, and community resource planning.
• Facilitates referrals for follow-up care.
• Demonstrates the ability to work effectively with clients of all ages and with diverse cultural needs.
• Develops and maintains a thorough knowledge of referral resources throughout the service area.
• Works closely with members of the interdisciplinary health care team in development of plan of care.
• Performs other related duties as assigned.
