About The Position
The position involves planning and implementing treatment and transition care plans based on a psychosocial assessment that is integrated into an interdisciplinary care plan. The role requires providing a range of social work interventions, including case management, counseling, crisis intervention, and advocacy, to assist patients and families in reaching their goals. The social worker must identify and recognize rapidly changing situations and quickly intervene to prevent or resolve emotional crises. Additionally, the role includes arranging, mobilizing, utilizing, and coordinating a comprehensive range of community resources to effectively meet the identified needs of patients and families. The social worker will take an active role in facilitating interdisciplinary communication regarding patients and families experiencing complex psychosocial issues. Education of patients, families, and healthcare team members about the social work role and pertinent patient care issues is also a key responsibility. The social worker may serve as a preceptor or field instructor for social work undergraduate training and will act as a patient advocate and liaison with physicians, families, and the community to ensure continuity of care. Documentation of psychosocial assessments, progress notes, care plans, and discharge/transition plans in the electronic medical record is required. Participation in quality improvement, research, and program evaluation initiatives aimed at improving healthcare services is also expected. Knowledge and compliance with MHealth Fairview and Social Work practice, ethics, and standards are essential, along with adherence to legal and regulatory laws, including mandated reporting requirements.
