About The Position

The Social Worker (Bachelor's) provides essential social work services and counseling for patients and their families, collaborating closely with physicians and healthcare team members. The role involves conducting assessments, making timely referrals for psychosocial evaluations, and ensuring that patients receive appropriate support throughout their care journey.

Requirements

• Bachelor's Degree in Social Work or related field from an accredited college or university.
• Current certification as a Licensed Baccalaureate Social Worker (LBSW) in the State of Tennessee upon hire.
• Qualified by education, training, or experience to work with adolescent, adult, and/or geriatric patient populations.

Nice To Haves

• Minimum 1 year of social work experience preferred.

Responsibilities

• Collect key information from patients, families, and community agencies as part of social work assessments.
• Determine priorities for social work intervention, focusing on discharge planning and patient self-determination.
• Provide crisis intervention services to enhance coping mechanisms for patients and families.
• Communicate with community service providers to identify high-risk patients requiring social work services.
• Assist patients in obtaining necessary services such as DME, outpatient therapy, and home care.
• Document assessments and recommendations within twenty-four business hours of referral.
• Coordinate discharge planning early in the patient's hospitalization.
• Maintain effective relationships with collateral agencies and include them in multidisciplinary team conferences.
• Utilize computer systems to enter and retrieve medical/patient information and generate reports.
