Requirements

• BSW in Social Work (preferred) or BA in Counseling or Psychology. MSW/MA preferred.
• Social Work license in State of WV
• One year experience with children preferred.
• Good verbal and written communication skills.
• Knowledge of community resources
• Knowledge of authorization of care, child welfare and juvenile justice processes
• Ability to work collaboratively with others
• Ability to assess patient/family needs for treatment and discharge planning purposes
• Customer service
• Treatment/discharge planning.

Responsibilities

• Treatment Planning
• Discharge/aftercare planning
• Utilization review, including submission of continued stay requests
• Chart reviews and medical record documentation, including reports
• Communication with patients, families, physicians, staff and external entities
• Conduct psycho-education and focus groups with patients
• Patient/family education
• Testifying at court and participating in MDT meetings. Transports youth.
• Provides additional functions as needed by program.
