  • Assist in providing education, in-service, orientation, and training to social work students, interns, or volunteers.
  • Complete timely assessments and documentation in resident medical records, including MDS assessments, admission notes, psychosocial assessments, and care plans.
  • Manage assigned caseload, including assessments, scheduled meetings, and follow-ups with residents and families.
  • Facilitate timely room and unit changes by notifying residents and preparing them for changes.
  • Attend mandatory in-service education programs to stay current on nursing home regulations.
  • Participate in interdisciplinary committees and program development as assigned.
  • Assist residents and families in meeting personal needs such as clothing and burial plans.
  • Facilitate effective communication between residents, families, and the interdisciplinary team regarding concerns and grievances.
  • Follow up with new admissions and families to help with their adjustment to placement.
  • Conduct environmental rounds to identify and address safety issues.
  • Communicate with the interdisciplinary team to meet resident psychosocial needs effectively.
  • Make referrals to mental health providers and facilitate psychiatric hospitalizations as needed.
  • Educate residents and families about advance directives and healthcare decision-making.
  • Identify residents who may benefit from palliative and hospice care services and make appropriate referrals.
  • Coordinate discharge planning efforts for residents and follow up post-discharge.
  • Address incidents of alleged abuse, neglect, or mistreatment promptly.
  • Provide counseling to residents on various issues such as adjustment, non-compliance, and bereavement.
  • 1 year of experience as a Social Work Assistant in Long Term Care is required.
  • Knowledge of discharge planning and care plans is essential.
