The social work assistant coordinates functions associated with patient information processing for the Child and Adolescent Partial Hospitalization Service (CAPHS). Assists and acts as a resource to clinical staff providing care to patients, ensure continuity of care, and support compliance with treatment. Monitors and ensures that all tasks assigned are completed in an accurate, efficient, and customer friendly manner. Performs functions associated with gathering and processing patient information for ambulatory care visits. Understand and have an awareness of potential presenting problems, mental diagnosis, and the unique needs of the patients and their families. This includes, but is not limited to the need for de-escalation, sensitivity, triage, and referral. Completes the responsibilities of reception, registration, e-Record task management, and program referral management via EMR referral work as well as other methods of incoming referrals. The social work assistant may participate and engage directly with patients in the therapeutic milieu, alongside licensed clinical staff, as needed. The social work assistant will collaborate with the primary therapists and community providers to facilitate timely access to services and support transition between levels of care; to provide access to concrete services that support treatment adherence.
