Requirements

  • Bachelor's degree in Social Work, Psychology, Sociology, or related Human Services field
  • One year full-time professional experience in a social services agency OR Two years full-time paraprofessional experience in a social services agency.
  • Valid Driver's License

Nice To Haves

  • Master's degree in Social Work, Psychology, Sociology, or related Human Services field
  • Previous experience in completing initial screening and assessment in one or more of the Adult Service Program areas: Adult Protection, Chemical Health, Mental Health, Housing, and Pre-Petition Screening.
  • Pursuant to Dakota County Policy, this position is designated as Safety Sensitive. "Safety-sensitive position" means a position, including any supervisory or management position, in which impairment caused by drug, alcohol, or cannabis use would threaten the health and safety of any person as defined in Minnesota Statutes Section 181.950, subdivision 13.

Responsibilities

  • Effectively engage individuals and their families to assess and evaluate needs and strengths, including safety and risk.
  • Determine with the client, and in collaboration with agencies, eligibility, need, and level of service for identified clients.
  • Provide immediate response in crisis situations. Maintain safety for clients and community. Develop and implement a safety plan for clients to maximize their safety and minimize community risk.
  • Develop and implement all or part of a service plan (as applicable) defining initial and ongoing measurable goals and objectives.
  • Provide, arrange, and coordinate direct services and advocacy to clients and families.
  • Facilitate, integrate, and mobilize agency and community resources on behalf of the client.
  • Evaluate, document, and monitor progress toward goals and objectives.
  • Research, maintain, and apply knowledge of relevant policies, procedures, and statutes. Provide training and education to others.
  • Complete agency, state, and federal required documentation as related to client activity according to mandated timelines.
  • Complete closure of case management activity with client based on resolution of established goals and/or successful integration to community resources. Terminate activity with client when client requests closure or when involuntary closure is ordered.
  • Maintain appropriate engagement and boundaries with clients and colleagues.
  • Demonstrates a commitment to support cultural diversity County wide, actively supports the County's inclusion, diversity and equity initiatives, and works to address the needs of diverse and underserved communities.
  • Perform other social services duties as assigned.
