The Acute Rehab Social Worker acts as the overall care coordinator of each person served, family/support system and collaborates with the multi-disciplinary team in order to achieve the desired outcomes defined in the individualized plan of care built by the rehabilitation physician. The social worker is knowledgeable about the acute rehabilitation program and is identified by the person served, the family and support system. The social worker performs activities to meet the needs of the persons served in a manner that optimizes the discharge planning process and facilitates post-acute rehabilitation continuity of care. The social worker undertakes a leadership role in advocacy, negotiation, mediation and brokering for the rights and entitlements of the persons served. The social worker provides counseling, education and support programs that enhance the ability of persons served/family to cope with physical, emotional, cultural, religious, lifestyle and environmental changes/concerns.
