About The Position
The Department of Obstetrics and Gynecology at SUNY Downstate Health Sciences University is seeking a full-time Social Worker III. The successful candidate will provide psychological assessment, support and initiate patient referrals for proper management. They will identify the psychological needs of the patients and ensure that these needs and services are addressed. The role involves collaboration with the Attending Physicians in the clinics for appropriate patient follow-ups and overseeing the MICI Program staff along with the logistical details. The Social Worker will function collaboratively with the MICI staff on a daily basis for patient-related updates, manage the Providers' referrals, complete the required MICI forms, and provide appropriate follow-up of high-risk patients for compliance measures. Additionally, they will document patients' information in the HealthBridge system.
