About The Position

As a Social Worker 2, you will provide a full range of professional social services and support for residents, clients, and resident representatives to plan, provide, and coordinate essential social services. As a member of an interdisciplinary care plan team, you will attend meetings, maintain accurate and complete written documentation, complete admissions, quarterly, and annual resident assessments, and develop person-centered plans of care. This role requires the ability to provide counselling services that will support residents and help them overcome behavioral and psychosocial obstacles. In addition, you will coordinate discharge planning and transfers, as well as offer consultation and education to other disciplines in developing behavioral management programs.

Requirements

  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (Commonwealth titles) (must have been employed in a Commonwealth social worker job title on September 8, 2008, with continuous Commonwealth employment); or
  • Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor's degree in social work or social welfare; or
  • Two years of professional experience providing direct public or private social work services, and a bachelor's degree in social work or social welfare; or
  • A master's degree in social work or social welfare.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
  • This position requires an active license as a Social Worker or Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.
  • You must meet the PA residency requirement.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require a medical examination.
  • This position falls under the provisions of the Older Adult Protective Services Act.
  • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
