About The Position

The Social Worker LMSW conducts psychosocial evaluations, provides individual and group psychotherapy, care management services and discharge planning. Develops and implements patient treatment plans and participates in treatment team conferences. Relies on education, experience and judgment to accomplish job. Works under supervision required by regulation and licensing board. Creativity and some latitude is expected to complete responsibilities.

Requirements

• Master's Degree in Social Work.
• Certification as a Masters in Social Work (or CSW) by the Louisiana State Board Social Work Examiners.
• BLS Certification.
• Crisis Intervention Training within 90 days of employment.
• Proficient in the English language, verbal and written communication skills.
• Basic computer skills.

Responsibilities

• Conduct psychosocial evaluations and provide individual and group psychotherapy.
• Provide care management services and discharge planning.
• Develop and implement patient treatment plans.
• Participate in treatment team conferences.
• Serve as the case manager for assigned patients.
• Coordinate the interdisciplinary assessment process.
• Complete psychosocial assessments within organizational guidelines.
• Conduct family meetings as necessary and directed by the physician.
• Complete treatment plan documentation and updates according to patient needs.
• Act as an information and referral source by developing cooperative agreements with community agencies.
• Participate in departmental and organizational quality improvement initiatives.
• Provide high quality counseling to staff members and facilitate staff support groups.
• Act as a clinical resource and mentor to health care team members.
• Participate in community outreach services as an educational resource.
