About The Position

The Community Forensics Services Department at the Forensic Aftercare Clinic located in New Orleans, LA. The Mission of ELMHS is to provide mental health evaluation, treatment, habilitation and consultation for adults. Programs include inpatient interventions, jail based evaluations, supervised group home and residential programs, forensic evaluation, care, custody, and aftercare; intermediate and long term psychiatric are in coordination with the total system of care in Louisiana The Forensic Aftercare Clinic is seeking a passionate Social Worker to deliver direct services to clients in Regions 1 and 10 including treatment planning, drug testing, coping skills and substance abuse groups, conducting home visits and assisting in transporting clients to and from inpatient treatment programs. Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Certified Social Worker (CSW) plus: A master's degree in social work. NOTE: Permanent status will not be granted in this job title without LMSW or LCSW licensure in Louisiana; Certified Social Worker (CSW) credential will not be accepted for granting permanent status.The official job specifications for this role, as defined by the State Civil Service, can be found here.

Requirements

  • Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Certified Social Worker (CSW)
  • A master's degree in social work.

Responsibilities

  • Provide on-going treatment including individual and group counseling, as well as treatment compliance monitoring that includes drug and alcohol screening.
  • Provide a comprehensive psychosocial assessment (mental health and addictive disorders) during admissions.
  • Coordinate all discharge planning activities including but not limited to managing/monitoring staff edits to support chart integrity and conducting and reporting on final chart audits.
  • Assist with records requests and review of said records, and follow-up with other providers in an effort to secure the information necessary to assess risk and treatment needs or gaps in care.
  • Present cases for staffing and documents any changes in client functioning with a goal of participating in quality assurance activities and service delivery standards that promote satisfactory client outcomes.
  • Participate in education trainings that are mandatory for accreditation and licensure of the facility.
