About The Position

Provides various social work services for older adult clients or clients who are potentially at risk, their families and caregivers. Responsibilities include conducting assessments, writing individual plans of care, individual counseling, facilitating support groups and conducting intakes. Assists callers or drop-ins seeking information and referral to various community services; conducts program tours; represents programs that focus on seniors at various community events and health fairs.

Requirements

Bachelor's: Social Sciences, Social Work, Gerontology or related fields or equivalent education/experience
2 years of recent relevant experience.
Knowledge of community resources.
Knowledge of individual and family development over the life span and the influence of cultural and spiritual values in health care.
General knowledge of illnesses, current treatments, and their physical and psychosocial effects on the individual.
Knowledge of applicable laws and regulations, government, and insurance benefits.
Excellent interpersonal skills and time management skills, ability to work effectively in a fast-paced environment with rapidly shifting priorities and competing demands.
Demonstrated ability to work independently, manage projects/tasks and deliver results on time.
Communication skills, both written and verbal.
