The Social Work Supports Coordinator uses person-centered principles to assess the medical, functional, psychological, financial, and environmental needs of participants. Works with participants (or caregiver) to develop a long-term care (LTC) plan and assists the participants to manage their care needs. Duties Include: The SC will assess the functional, psychological, financial, and environmental needs of potential Care Management participants and participants in the Veteran Directed (VA) program. Work with the participant (and his/her selected designee) to develop a person-centered plan; assist and support the participant to manage their care needs. Provide LTC options and strategies including the eligibility, frequency, duration, and financing of services. Educate participants/caregivers regarding the participant's health status, health promotion, and risks and benefits of treatment options. Monitor and reassess participants and quality assurance activities. Accommodate daytime, evening and weekend hours when needed by the department.
