About The Position

Under the supervision of the Specialized Health and Social Services (SHSS) Program Manager (PM), provides direct supervision and performance evaluation to unit comprised of public health social workers. Develops job descriptions and performance standards. Participates as a member of the regional leadership team. Serves as subject matter expert for various program service areas. Provides training, consultation, quality assurance, and technical assistance to providers, contractors, and regional staff. Ensures that appropriate referrals are made by staff. Assists SHSS Program Manager with planning, implementing, monitoring, and evaluating social work programs in Public Health Region (PHR) 07. Develop regional program standards and procedures in collaboration with regional committees and work groups. Plans, assigns, and supervises the work of others. Oversees the hiring, orientation, and training of public health social workers, along with promoting professional growth and development of staff. Effectively manages programmatic changes and routinely takes the initiative to find solutions to identified challenges. Provides program information to community-based organizations, medical professionals and the general public. Functions as a vital member of the regional Incident Command structure establishing guidance and responding to public health emergencies. Performs other duties as assigned. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgment.

Requirements

  • Knowledge of: social work and case management principles and best practices; children’s programs such as Medicaid, CSHCN-SP, CPW, DFPS/CPS/Foster Care, SSI, NBS, NBHS, Lead, ECI, and CHIP; quality assurance and technical assistance policies and procedures; community resources on a local, state, and national level; the impact of social determinants of health on populations served.
  • Skill in: effective management methods, including team leadership and facilitation, providing supervision, and work product monitoring; delivering presentations and trainings on comprehensive social work and case management principles and best practices, policy application, and program processes; providing guidance, support, and programmatic oversight to staff; use of electronic mail, internet, databases, and Microsoft Office products; conducting telephonic and in-home intake and family needs assessments; developing service plans; conflict resolution, mediation, and problem solving; planning, prioritizing, and organizing workload; compiling reports; effective independent decision making; leading team members to accomplish a variety of program goals and deliverables.
  • Ability to: communicate effectively, verbally and in writing, with diverse and multi-generational populations; thoroughly assess, analyze, summarize, and evaluate outcomes; tactfully and objectively review and provide feedback on documentation, assessments, policies/processes and performance; compile, analyze, evaluate, and synthesize technical data and qualitative written reports for administrative use; establish and maintain effective working relationships with all levels of staff in an interdisciplinary setting; interpret and apply program policies and procedures; submit timely work activity, travel, and time reports; maintain thorough and accurate documentation of all planning, performance, and counseling session meetings with direct reports; maintain confidentiality; plan and manage multiple workloads and assignments to meet deadlines; follow DSHS and HHS Policies and Procedures; adhere to HHS Work Rules and the National Association of Social Workers Code of Ethics; function independently in the daily performance of professional duties; provide own transportation to perform work duties.
  • Must be a current unrestricted Licensed Social Worker in the State of Texas.
  • Must possess a valid Class C Texas Driver's license or equivalent license from another state.
  • Bachelor's degree (minimum) from a Council on Social Work Education accredited college or university
  • A minimum of 1 year of leadership experience as a team lead, supervisor, or trainer is required.
  • A minimum of 5 years working in a governmental, public health, or human services setting performing a social worker role (case management, healthcare navigation, needs assessments, and/or case reviews) is required.

  • Experience with Personal Care Services, Community First Choice, Children with Specialized Health Care Needs, and/or Texas Health Steps is preferred.

Responsibilities

  • Provides direct supervision of public health social workers/case managers and administrative support staff (as assigned) within designated unit.
  • Works collaboratively with program management and other program supervisors to plan, develop, implement and monitor program operations to ensure compliance with program and agency policy, including: gathering data; providing written and/or oral feedback; and conducting internal quality assurance activities.
  • Takes a leadership role in problem solving.
  • Completes the selection and promotion process according to agency policy and procedures.
  • Monitors and directs program/agency orientation, education, licensure requirements, and staff adherence to agency human resource and program policy and procedures.
  • Oversees day-to-day program operations for the public health social work/case management aspect of the PHR 7 SHSS program.
  • Meets with individual direct reports on a routine basis to share feedback on their performance.
  • Develops professional performance plans with all direct reports and completes individual performance evaluations that accurately reflect their performance according to human resource policy and procedures.
  • Recommends rewards and disciplinary actions for assigned staff.
  • Manages employee time and leave to ensure appropriate staffing for efficient and effective program operations.
  • Conducts routine unit meetings.
  • Serves as subject matter expert in the following program service areas: Children with Special Health Care Needs Services Program, Newborn Screening, Case Management for Children and Pregnant Women, Personal Care Services, and Community First Choice.
  • Performs Cursory Reviews of case management assessments and documentation.
  • Responds to inquiries from Central Office and regional management.
  • Participates in CSHCN-SP contract management and case management provider relations and represents DSHS at community events.
  • Assists with monitoring and directing activities of direct reports during special projects or assignments.
  • Performs other duties as assigned.
  • Supports regional public health initiatives and serves as a member of the Regional Leadership Team.
  • Attends trainings relevant to licensure requirement and continued development of professional social work knowledge and skills.
  • Performs other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation . Such participation may require an alternate shift pattern assignment and/or location.
