The Brooklyn Transitions Program is a specialized borough-wide prevention program for families challenged by substance use and/or mental health issues. The Social Work Supervisor ensures that clinical casework and case management services provided by the staff meet or exceed expected standards of professionalism and that the prevention program complies with agency and Administration for Childrens Services (ACS) policies and contractual commitments.
