OPEN will be a tri-campus hub for triage, assessment, and evaluations to assist adult patients finding the right mental health care at the right time. This hub will unite existing staff across campuses to centralize and streamline access while decreasing redundant, unnecessary, and time-intensive work through optimization of our electronic medical record and with the use of innovative technologies. OPEN will support our strategic initiative to improve access to our outpatient programs, provide transparency to available services and treatment within our system, make rapid and appropriate recommendations for services to a larger outside ecosystem while supporting Ambulatory Services’ mission to provide tertiary care to the most acute patients. The Social Work Resource Liaison will help support Outpatient Psychiatry Evaluation and Navigation (OPEN) Hub in delivering high quality patient care by reviewing and completing referrals, interfacing directly with patients, coordinating evaluations, and collaborating with various team members in a fast-paced, evaluation-focused environment. As part of this innovative team, you will participate in interdisciplinary daily triage meetings and support patient disposition planning by locating outside resource information and responding to inquiries about a broad range of community services, eligibility requirements and application procedures. This role is ideal for someone eager to advance their triage and disposition skills and be a part of a transformative model of outpatient mental health care. This is a full-time day shift position at New York-Presbyterian, physically based at Westchester Behavioral Health. This position is Monday through Friday day shift, 9:00 am- 5:00 pm (hybrid in person/virtual possible).
