Deliver quality patient care by assisting patients in accessing treatment through preliminary screenings, coordinating initial evaluations and collaboration with various team members within the outside of NYP. Support the team through patient discharge planning by locating resource information, and rapidly responding to inquiries about a broad range of community services, eligibility requirements and application procedures. Additional responsibilities include occasionally escorting patients to appointments in the community and sorting and arranging donated patient clothing. Shift time: Monday - Friday from 8:30-4:30

Requirements

  • Basic Life Support Certification
  • Experience with or willingness to learn word processing, spreadsheets and other PC software
  • Basic knowledge of human development and knowledge of basic Social Work skills
  • Personal qualities such as capacity to learn, sound judgement, good interpersonal skills, assertiveness and organizational ability are important
  • all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP
  • NYP will not reimburse for travel expenses

Nice To Haves

  • Bachelor Degree
  • Experience working in a hospital setting
