About The Position

The Social Work Patient Care Coordinator is responsible for improving the quality of patient care as a member of the multidisciplinary patient care team. This role involves identifying and facilitating the social work service needs of patients and families in both acute care and outpatient populations. Key responsibilities include conducting psycho-social assessments, providing information and referrals, performing high-risk screenings, crisis interventions, resolutions, and early discharge planning. The coordinator facilitates the discharge planning process throughout the patient's continuum and works to promote optimal clinical, financial, and satisfactory patient outcomes. The position requires effective communication with patients, families, physicians, and other members of the patient care team, as well as community representatives. Additionally, the coordinator supports change, challenges current healthcare delivery models, and advocates for more efficient methods for delivering social work services. Medical specialty care coordination and coverage of other clinic areas may also be assigned.

Requirements

  • State of Ohio, Licensed Social Worker (LSW) required.
  • Master's Degree in Social Work required.
  • Licensed Independent Social Worker (LISW) preferred.
  • Previous social work experience in a healthcare setting required.
  • Competent in computer skills (Word, Excel, Internet, and email) required.
  • Excellent written and verbal communication skills required.
  • High level of integrity in dealing with confidential data required.
  • Flexibility in daily assignments as needed required.
  • Adherence to principles of infection control as defined by risk assessment required.
  • Weekend/holiday rotation required.

Responsibilities
