Requirements

  • Licensure as a Licensed Social Worker in Minnesota and North Dakota.
  • Strong written and verbal communication skills.
  • Ability to work independently, balance multiple priorities, and think critically.
  • Proficiency with electronic medical records and comfort learning new software systems.
  • Valid driver's license and insured personal vehicle.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with clients, families, and team members.

Nice To Haves

  • Hospice social work experience preferred.
  • CPR certification (or willingness to obtain within 6 months of hire).

Responsibilities

  • Be a Champion of Collaboration - Partner closely with the education team to shape, test, and refine new process rollouts before they hit the field, ensuring practicality and effectiveness from the front-line lens.
  • Support Clinical Excellence - Serve as a clinical resource to peers, offering guidance on evidence-based psychosocial interventions, care planning, and evolving regulations and standards.
  • Promote Quality & Innovation - Gather feedback from team members across regions, using data and insights to help develop meaningful improvements in service delivery and education.
  • Directly Support Clients & Compliance - Conduct psychosocial assessments, provide emotional support, and guide patients and families through end-of-life transitions with dignity and compassion.
  • Maintain meticulous client records and billing documentation.
  • Ensure understanding of all payer sources and authorizations and uphold regulatory standards in all client interactions.
  • Strengthen Communication - Be a consistent liaison between social work staff, the interdisciplinary team, and leadership—bringing voice to the field while ensuring aligned care delivery.
