The Impact You Can Make Team Impact The Snuggery Discharge Planner (DCP) will intervene with Snuggery service-line patients and families who have complex psychosocial needs, require assistance with social programs and funding sources, and qualify for community assistance from a variety of special funds and agencies. In addition, the Snuggery DCP collaborates with OB/GYN and Pediatric providers to coordinate and facilitate transitions of care for complex patients. How You Will Fulfill Your Potential Responsibilities Assesses patient’s and family’s psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to pregnancy and ability to cope. Intervenes with patients and families regarding emotional, social, and financial consequences of pregnancy/birth; accesses and mobilizes family/community resources to meet identified needs. Provides intervention in cases involving child abuse/neglect, domestic violence, substance use disorder, institutional abuse. crisis, and sexual assault. Provides bereavement support to patient and families. Meets with patient/family as soon as possible after admission to assess, evaluate, and identify discharge needs. Provides support and information, as needed. Connects with patient/family as soon as possible when consulted prenatally/antepartum in order to facilitate proactive complex discharge planning. Participates in discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers. Deals with families exhibiting complex family dynamics that impact directly on patient care and discharge. Collaborates with maternal & pediatric providers and multidisciplinary team to develop, plan, and facilitate a safe discharge plan while adjusting as maternal and newborn condition changes throughout hospitalization. Collaborates with Risk, Quality, and the interdisciplinary team as need to foster safe outcomes. Communicates with Care Continuum Director/Supervisor regarding the discharge planning when needed. Provides coordination when significant or intensive community resources are necessary to achieve desired treatment outcomes. Educates patient/family and provider regarding post-acute options and addresses issues of choice. Provides intervention in child abuse/neglect, domestic violence, guardianship (temporary/ permanent), foster care, adoption, mental health placement, advance directives, adult/elderly abuse, child protection and sexual assault. Follows the expectations of the department’s social work role and responsibility grid Demonstrates the knowledge and skills necessary to provide appropriate care in consideration of the growth development, and social needs of neonatal, pediatric, adolescent, and adult patients (parents). Relays any barriers or concerns regarding any aspect of their role to the Sr. Director or Supervisor Continuously pursue excellence in meeting the needs and expectation of all customers (patients, families, inter-disciplinary team members, payors, screener, liaisons and outside services and agencies. Initiate and/or coordinate plan of safe care for families who have substance use disorder. Acknowledges issues of diversity and adjusts practice accordingly. OTHER FUNCTIONS 100% of Annual mandated programs are to be timely. Maintains and submits weekly activity statistics. Identifies their own learning needs and will actively seek opportunities for on-going clinical growth through participation in educational programs, current literature, in-services, meetings, and workshops. Works closely with Director and/or Supervisor on Snuggery departmental coverage needs. Works Monday through Friday, however, will need to be on-call for department coverage on weekends and holidays. Performs similar or related duties as assigned.
