About The Position

Sampson Regional Medical Center is hiring a Social Work Discharge Planner to join the Continuing Care team! The selected candidate will provide a variety of casework services and supportive counseling to assist patients and/or families to return to optimum functioning capacity. Additional responsibilities include devising and implementing discharge plans to facilitate continuity of care. Requires knowledge of community resources, Medicare, Medicaid, insurance coverage, and ability to work closely with patients and families, nursing staff, medical staff, other departments and community resource personnel.

Requirements

• Bachelor of Science, Bachelor's in Social Work preferred.
• 2 years' experience in Social Work preferred.
• Knowledge of patient referral system and Home Health programs.
• 4 to 6 months on the job training required to become familiar with hospital and departmental policies, procedures, routines, federal and state regulations.
• Must be computer literate and keep abreast of constantly changing regulations.
• Must attend development activities to enhance job performance.
