Pay Information: Internal Hires: Pay rate will be determined based on applicable personnel rule or union contract terms. External Hires: Step placement and rate of pay will be determined based upon relevant experience. SWCM I starts at $35.97 per hour. SWCM II starts at $39.86 per hour. SWCM III starts at $42.28 per hour. Job Summary: SWCM I: Within the Case Management Department this position provides social work and discharge planning in predictable situations using established policies, procedures, ACMA guidelines, and NASW code of ethics. This position requires direct to regular supervision, but is able to recognize attributes (i.e., isolated traits, objective measures or tasks) of a social work situation without situational experience. This position may participate in other hospital committees and teams as appropriate. Incumbents working at this level are generally assigned to one area of the hospital. This is the trainee/entry level with direct supervision from an experienced Case Manager. Assignments are meant to orient the incumbent to BRH Case Management services, protocols, techniques, and allow for on-the-job training in preparation to advance to the journey level. SWCM II: Within the Case Management Department this position provides social work and discharge planning using established policies, procedures, ACMA guidelines, and NASW code of ethics. This position plans and implements complex individualized social work care, with intermittent supervision. Social Workers at this level are able to manage routine and emergency care. They assume basic leadership responsibilities at times and provide mentoring of Social Work Case Manager I's when requested by the Director of Case Management. This position could make case assignments if requested and they are proficient in a primary area, but are able to float to other locations in the hospital. This is the journey level with intermittent supervision. Incumbents may independently plan and implement social work care, provide mentoring of SWCM I's, and are able to float to other locations in the hospital if needed. SWCM III: This is a clinical case management position that provides social work case management and behavioral health services throughout the organization. The employee must be trained as a master's level social worker and be able to practice in the state of Alaska and maintain all licensing requirements; complete annual educational and HR requirements; practice professional ethics including the maintenance of appropriate boundaries. Documentation needs to meet Bartlett Hospital and CMS standards while using multiple medical records systems depending on service location. Collaboration with partners along with frequent communication is essential. Case Management services will include social work services, development of care plans, and discharge planning using established policies, procedures, ACMA guidelines, and NASW code of ethics. Manage routine and emergency care. Assume basic leadership responsibilities. Mentoring of Social Worker I CM when requested by Director of Case Management. This position may be assigned to any of the units served by CM, including medical services, Home Care Services, Mental Health Unit, the Emergency Department, or Rainforest Recovery Center, and will float between these units as needed. Behavioral Health services will include behavioral health assessments and intervention services throughout the hospital units. This is the advanced level with limited supervision. At this level, incumbents are experienced and hold a Master's Degree in a relevant field giving them the ability to independently plan and implement social work care for the most complex cases, provide mentoring of SWCM I's & II's, and assigned to float to other locations in the hospital if needed. Positions at this level also conduct clinical evaluations and assessments.
