About The Position
Responsible for direct social work case management services to the participants of the StayWell Senior Care program. Provides social services support as: participant screening, case management, counseling, and referral.
Requirements
- Master's degree from an accredited School of Social Work
- Possess individual therapy skills for geriatric participants, analytical thinking and problem solving abilities
- Willing to conduct home visits to private homes of participants in Randolph, Montgomery and Moore counties
- Only act within the scope of authority to practice
- Meet standardized set of competencies for the specific position description established by StayWell Senior Care and approved by CMS before working independently
- Have valid NC driver's license and vehicle
- Minimum two (2) years' experience in a health related area
- Minimum one year experience working with the frail elderly population
